Embalmed:
1. Death certificate
2. Health certificate
3. Notarized embalment certificate
4. There is a $30.00 fee per document (money order to the consulate of panama)
5. Send a self addressed pre paid envelope to return your documents.
6. Original documents are required.
Cremated:
1. Death certificate
2. Notarized cremation certificate
3. There is a $30.00 fee per document (money order to the consulate of panama)
